Violations included dirty ice makers, an ice scoop inside ice, cleaning supplies near food and work surfaces, no thermometers, no soap, no paper towels, dirty sinks, employees not wearing hair or beard restraints, rags were not stored properly, dirty floors and baseboards and the mop sink area has mold or mildew.
