Papa Murphy’s Pizza located at 4020 Weber scored a 99.

Violations included a dirty slicer, no food manager on duty, no thermometer, an expired health permit, employees not wearing hair restraints, a dirty men’s room and the urinal was out of order.
The back door sweep, a/c vent cover and light fixture cover need to be repaired.
