Man dies in fatal fall at Valero West Plant

A man is dead following a workplace accident Wednesday morning. 

It happened around 10 a.m. at the Valero west plant refinery located at 5998 Up River Road. 

At this time, not many details have been released.  According to the Medical Examiner's Office, a scaffolding worker fell about 50 feet to his death.

The victim has been identified as 52-year-old Ezequiel Guzman of San Antonio who was working for Brand Scaffolding out of San Antonio. 

As to why or how or he fell, Guzman's death remains a mystery.  This is the second work related death this week that OSHA is investigating. 

This past Sunday, a man reportedly fell to his death at the Interstate Grain Corporation located just down the street from Valero. 

That victim was identified as 21-year-old Merejildo Ramos. OSHA officials say he died from a fall as well, and according to the Medical Examiner's Office his toxicology results are pending.
